The prevalence of medical errors in healthcare has awakened the healthcare system to the challenge of reducing the number of adverse events in hospitals. HARI and hospitals in RI developed patient safety teams to address error prevention. There are several foundational blocks that have led Rhode Island hospitals to innovative patient safety through HARI's efforts. Some HARI efforts include the Zero Harm Collaborative, the High-Reliability Organization (HRO) to reduce preventable harm conditions, the Uniform Surgical Protocols to prevent wrong-site surgeries, along with improving workplace safety. These efforts were developed to create safer environments for patients and healthcare workers in Rhode Island. HARI and its members value reducing harm and improving patient care throughout Rhode Island.
